Community Outreach


This service is for people living in the community either with their families or independently. As with all of our services, we aim to help individuals develop strategies and life skills, such as budgeting, cooking and cleaning, which enables them to reduce the requirement of formal support.

Support plans will be developed with the individual to maximise choice and based on an in-depth assessment of need. 

Each individual has varying hours of support depending on their specific needs which range from 7 hours per week to 24 hours per day, 7 days a week. Support is given to maintain dignity and respect, build confidence and self-esteem, thereby ensuring that the person is empowered to access desired activities and maintain and develop their skills at all times.
