Learning Disabilities Primary Care Health Liaison Team

Description

We help people with things to do with Primary Healthcare.  "Primary Healthcare" means health services in the community, like: GPs; dentists; health centres; opticians; chiropodists.

We work with health services - helping workers support people with learning disabilities.
 
We work with people with learning disabilities - helping people get better access to primary healthcare services.
 
We work with carers - helping them learn about different health problems and where to get help.
 
We also:
  • do group work on different health topics
  • talk to you about how to use your Health Action Plan
  • work with GPs to make sure people are on the learning disability register and get an Annual Health Check
  • make sure that people have health information that is easier to understand
  • make sure that health services think about people with learning disabilities when they make plans
  • make sure that people with learning disabilities are given fair access to health services
  • work with the Acute Liaison Team.  They support people with learning disabilities who have to go into hospital
